Purpose:   It is recognized that this web site information; specifically the ... Survey / Questionnaire Forms could not address all of the issues of the parents or schools, so a " blank free form survey page" called the BLANK  Survey / Petition / Poll Form was produced to allow parents and citizens to establish additional "talking points" or "action points".  The  BlankSurvey / Petition / Poll Form  can be used in conjunction with the list below of: "Subjects" for the Blank Survey Form.  The Blank Survey / Petition / Poll Form  can also be used by itself with your own "hot topic" that addresses the immediate need.  It is hoped that this endeavor might permit an effective method for redress or recourse.

Please also note that various topics in the  Religious Guidelines from the Federal Department of Education are not all addressed below; but, you should be encouraged to pursue these other "topic(s)"as needed.

Recommendations:    Review the School Survey / Questionnaire Form to see if it addresses your higher priority concerns and your intended audience and then use it to:

• determine the overall interest level and
• prioritize each topic by its initial interest level and
• determine the volunteers' availability and potential time investment.
• or use it as a guide to design your own Questionnaire(s).

Instructions for this page:   The "Subjects" for the Blank Survey Form

• Please review the subject(s) and/orprograms listed below of interest to you.
• Then select one subject heading or one item for the survey/petition/poll.   Modify it, rewrite it, improve it for your particular situation and your intended audience.
• Do your homework on the subject and the program so that you are knowledgeable about how best to use the petition(s) and what to do with the responsive information.
• Have a plan for follow up like a "Survey Results" mail out or promote a meeting date and place.
• Print out the "Blank Survey / Petition / Poll Form" and type or print your Subject in the space provided.   Duplicate the petition forms and mail them out or distribute them to your volunteers and collect the information.
• Be advised that a typical survey response by mail will generally range between 1%-3% anything over this indicates a very high interest level.
• If your "petition subject" falls under a particular law or requires a specific process to be legal like a recall of an elected official, or enactment of legislation or law, then get some help from legal council.

Whereas no other reading program produces results like phonics which is mastery of only 77 distinct vowel sounds and should be on par with learning to count to 100, be it resolved that I/we, the undersigned,
